Our New Self Pay Membership Program

Our Packages:

Offering 3 different packages you can choose what’s best for you!

  • Package A

    • One annual physical
    • Two in-person office visits for sick visits or standard follow-ups
    • $450 per year
    • $37.50 per month

  • Package B

    • One annual physical
    • Three in-person office visits for sick visits or standard follow-ups
    • $500 per year
    • $41.66 per month

Package C

  • One annual physical
  • Four in-person office visits for sick visits or standard follow-ups
  • $600 per year
  • $50 per month

Additional Visits

Members who need care beyond the visits included in their plan may receive discounted pricing on their first two additional visits:

  • Additional in-person visit: $60
  • Tele-visit: $40

Services Not Included

The membership fee does not include:

  • Laboratory testing
  • Imaging, including X-rays, ultrasounds, CT scans and MRIs
  • Certain injections or procedures
  • Medications
  • Specialist visits or referrals
How Membership Payments Work

Each membership is a 12-month financial agreement for the full annual price of the selected package.

  • The annual fee is divided into 12 monthly installments
  • A valid credit or debit card must remain on file
  • Payments are automatically processed on the first day of each month
  • Office appointments may be scheduled after three days of enrollment
  • Annual physicals may be scheduled after 90 days of enrollment

    All included services must be used during the 12-month membership period. Unused visits do not roll over or convert into account credit.

Missed Payments

If a monthly payment is missed, the patient will receive written notice and will have 30 days to contact the office and resolve the payment. Membership services will be suspended until the account is brought current.

Failure to resolve the missed payment may result in termination of the membership. Because this is an annual agreement, termination does not remove the patient’s responsibility for the remaining annual balance.

Important Disclosure

This membership is not health insurance.

Harris Internal Medicine will not submit insurance claims for primary care services covered by the membership. The membership does not replace health insurance, workers’ compensation coverage or an employer’s legal insurance obligations. This self-pay membership is not available to Medicare or Medicaid patients.
